A case of adult T cell leukemia/lymphoma with CD 4+8+ subset is reported. A 78-year-old woman was admitted to our hospital because of bilateral cervical lymphnode swelling. The aspiration cytology showed class V suggesting malignant lymphoma, and extensive infarcts were seen in biopsied specimens. Although the histological classification was unclear, we assumed malignant lym-phoma at clinical stage II A. Cervical lymphnodes disappeared after radiation therapy for whole neck, but abdominal CT revealed swollen superior mesenteric lymphnodes which were absent at the time of admission. Despite the administration of combined chemothrapy, the lymphnodes grew rapidly. Marked leukocytosis with abnormal cells appeared soon after celiotomical biopsy, and she died of multiple organ failure. Histological diagnosis was non-Hodgkin's lymphoma of diffuse mix-ed cell type. Surface markers of the abnormal cells from peripheral blood were CD 3+4+8+. Serum anti-HTLV- I antibody and monoclonal HTLV-I proviral DNA were also positive. She was diag-nosed as having adult T cell leukemia/lymphoma.
