The relationship between plasma concentration of atrial natriuretic peptide and echocardiographic measurement of hemodynamic function.

The relationship between plasma concentration of atrial natriuretic peptide (ANP) and hemodynamic parameters by echocardiogram was studied in 143 patients. Following results were obtained. 1) Plasma concentrations of ANP were well correlated with left atrial dimension (LAD; r=0.46, p<0.001), left ventricular dimension in diastole (LVDd; r=0.37, p<0.001), or left ventricular ejection fraction (LVEF; r=0.28, p<0.01) in all of the patients. 2) Patients with atrial fibrillation (af) had significantly higher ANP level and larger LAD than those with sinus rhythm (SR). 3) Plasma NAP was highly correlated with LAD (r=0.37, p<0.001), LVDd (r=0.46, p<0.001) or LVEF (r=-0.43, p<0.001) in patients with SR. In contrast, there were no correlations in patients with af. These results demonstrated that ANP levels in patients with SR well correlated with hemodynamic datas, but not in those with af. 4) LVEF correlated well with LVDd (r=0.53, p<0.001) but not with LAD. These datas support that increased amounts of ANP are released from the left ventricle into the circulation in patients with left ventricular dysfunction.
