Three cases of herpes simplex encephalitis presented with various types of aphasia.

Three adult cases of herpes simplex encephalitis were reported. Case 1 (49, male) was referred to our hospital with mild disturbance of consciousness. He was already totally aphasic and CT scan revealed low density area in the left temporal lobe. CSF studies showed mononucleocytosis. With the 10-day administration of aciclovir, he survived but with severe memory disturbance and disorientation. Case 2 (41, male) and case 3 (36, male) had gnomic aphasia and pure word deafness on admission, respectively. Their CT scans were normal and lumbar punctures showed mononucleocytosis. Immediate start of the treatment using aciclovir was very effective for both cases and they were discharged without neurological deficit.MR images of case 2 revealed small abnormal region on left medial temporal area, which was not detected by X-ray CT.Immediate diagnosis and start of therapy in the very early stage of the disease are highly recommended to herpes simplex encephalitis which might be potentially fatal and may cause severe deficit of memory and emotion.
