RA is a progressive disease with remission and relapses and therapeutic strategy to cure the disease is still under investigation. Since most of the patients are female, usually housewives, the disease gives the impact upon the total functioning of the patient in the interrelationships with family and community. The earlier establishment of the RA care system in the local community is desired as a supportive social network with the help of family doctors, health stations, etc.The clinical feature of the disease is its systemic nature with multi-organ involvement, though its main lesions are in joints. Starting from smaller peripheral joints, the disease progresses to many joints, and symmetrical involvement is the most typical feature of the disease.Objectives of therapeutic modalities to manage the disease are to cease the inflammation, to relieve pain, to preserve physical functions of joints and muscles, and to prevent deformities. To achieve this goal, conservative treatments such as bed rest and exercise are recommended. Anti-inflammatory agents are the drugs of choice, and though they are efficacious, drugs must have high torelance and less adverse effect for a long-term administration.In recent years, gold or α-penicillamine is often used in treating the patients with RA, as these drugs might change the natural course of the disease and even cure the disease. Despite the possibility of side effects, these agents are important especially in severe cases such as those with progressive bone destructions.
