A low birth-weight infant who weighed 1, 300gm at birth (gestational age: 30w) was referred for severe abdominal distension. A radiograph of abdomen revealed massive free air in the abdominal cavity. Immediate laparotomy showed the inflamed terminal ileum with single perforation and multiple ballooning-out lesions. Exteriorization of perforated site and double barreled ileostomy were performed. Postoperatively she needed respiratory and nutritional support. Eighteen days later, closure of ileostoma was done successfully.Several factors contributed to such good result in this case are discussed.
