Although the treatment of heart failure in acute myocardial infarction is still controversial, a flow-directed balloon catheter was developed by Swan and Ganz in 1970 by which hemodynamic data can be easily obtained. In 1978 Forrester and his associates divided cardiac status into four subsets according to hemodynamic changes and suggested a program of treatment for each subset. Cardiologists in general have accepted Forrester's theory and incorporate it into their practice. In particular, our staff has reviewed the literature and our own experience and found for patients with heart failure in acute myocardial infarction immediate initiation of hemodynamic monitoring to be of great importance; implemented by appropriate therapy. We also evaluated the use of cardioactive drugs such as digitalis, cathecolamines, diuretics and vasodilating agents and mechanical assistance such as IABP.In summary, then, early and systematic assessment as well as a therapeutic program (which may include surgical treatment) is important for patients with heart failure.
