On the Indication of Pneumonectomy for Severe Pulmonary Tuberculosis

From the studies on pneumonectomy for severe pulmonary tuberculosis, the author concluded as following:1) Even though we may find a tuberculous lesion in the lung which was deemed healthy, pneumonectomy on the diseased lung can neverthless be advantageous. The indication is determined by the two conditions: a) if the size of the tuberculous lesion is no larger than 2cm in diameter, b) if the lesion is not a cavity. The post-operative prognosis is favorable.2) When the lesions are resistant to SM, INH and PAS, the post-operative complications occur more frequently than with non-resistant. Therefore determination of the indications must be more carefully done.3) As to the lung function, when the % vital capacity of the less diseased side is more than 80, and the % vital capacity of the more diseased side is less than 50, this case is recognized as an indication for pneumonectomy. On the other hand if the former % is 60-8O then it is desirable latter % be less than 20.Namely, the indication of pneumonectomy must be determined under the consideration of not only the lung function of non-operative side but the remained lung function.
