On the Mycobacterium Tuberculosis in Lesions of Resected Lungs

Bacteriological and streptomycin resistance tests were carried out on 190 lesions from 75 cases of lung resection.1) Of the 75 cases, 88% or 72.1% of the 190 lesions were positive for tubercle bacilli.2) 77.5% of the cultures of the lesions from those of positive sputa before operation were positive. Of those who had negative sputa, only 38.5% were positive.3) By classified lesions, 75.8% of cavity cultures, 76.8% of the concentrated cavity cultures and only 35.6% of the abcessed caseous lesions were respectively positive. Those of positive smears with negative cultures were 40.4%.4) By size, 70.6% of the cultures from lesions over 1cm in diameter were positive, while only 35.5% of those under 1cm in diameter were positive.5) No marked core lation was found between the pre-operation chemotherapy and bacteria found later in the lesion. Chemotherapy had no influence on cavities and concentrated cavities, however, in case of abcessed caseous lesions, chemotherapy gave more negative cases and cases of positive smears with negative cultures were concerned with the effect of chemotherapy.6) Increased dosage of streptomycin before operation not necessarily was higher incidence of negative bacteria in lesions. The more dose of streptomycin before operation in duced higher resistance of the bacteria to streptomycin, and in cases where over 30g was administered extremely high resistance was found.7) Bacilli in cavities and concentrated cavities were found to be resistant to streptomycin though it is hard to say that only those found in the cavities were exclusively highly resistant.
