The arthropathy is most commonly seen in SLE and is one of the most important symp-toms among many organs involved. Articular manifestations in 33 patients with SLE were studied clinically and immunologically. Twenty-two of these were used for a case-control study matched for sex and age concerning joint symptoms (arthritis and/or ar-thralgia) at the time of diagnosis in comparison with 22 patients with RA. Joints were impaired in 91% of patients with SLE during the course of the disease and 73% of them had arthritis. Fifty-five of patients with SLE had joint symptoms at the time of diagnosis and they had higher incidence of over 3TC of body temperature than patients with RA (p<0.005). Incidence of wrists involvement was significantly lower in SLE than in RA (p<0.05).The patients with SLE whose duration of joint symptoms was longer than 8 weeks were found not infrequently, and some cases developed peripheral neuropathy and were confirmed to have vasculitis histopathologically. Correlation between long-standing arthropathy and high titre of rheumatoid factors was found (p<0.025).Anti SS-A antibody among anti NAPA (nuclear acidic protein antigen) antibodies seemed to be present frequently in SLE patients with arthritis (p≈0.05). Thermography showed that arthralgia in SLE is a suggestive symptom of joint inflammation.One case with SLE had erosive arthritis and fulfilled the ARA criteria of RA.In conclusion:1) In contrast to RA, it is important that when we see arthritis and/or arthralgia in patients with SLE, we must pay attention to;(1) whether they have fever or not;(2) distribution pattern of joint involvement, particularly whether wrists are involved or not;(3) duration of jioint symptoms and their reactivity to steroids;(4) whether they have vascu-litis or not;(5) significance of rheumatoid factors in both SLE and RA.2) Vasculitis and rheumatoid factors are supposed to be related to the nature of arthro-pathy in SLE, and the relation between anti SS-A antibody and SLE arthropathy should he further investigated prospectively.3) Hand-thermography showed that even arthralgia in SLE might indicate the presence of joint inflammation.
