Sympathetic Blockade Improved Motor Dysfunction in Patients with Posttraumatic Cervical Syndrome

In two cases of posttraumatic cervical syndrome, motor dysfunction was markedly, but temporarily, improved by sympathetic blocks. The patients were 65- and 34-year-old males who suffered from cervical injuries one year and 4 months ago, respectively. They complained of reduction of grip strength and clumsiness of fingers associated with slight pain. Hypesthesia, lower skin temperature and slight muscle atrophy in the affected upper limbs and accelerated patellar tendon reflexes were recongnized. Their CT and MRI of the cervical region revealed compression of the spinal cord. Orthopedic surgeons suggested that there was no indication for surgical treatments. Physical therapy and cervical traction did not improve their muscle weakness and clumsiness of fingers. Stellate ganglion block with local anesthetics and thoracic alcohol sympathetic ganglion block improved markedly, but temporarily, the motor dysfunction. Although the mechanisms were unknown, the sympathetic nervous system might be partly related to motor function in patients with posttraumatic cervical syndrome.
