Percutaneous Diskectomy for the Pain Treatment of Cervical and Lumbar Disk Hernias

Objective: By the first week of June, 1994, percutaneous diskectomy (PD) had been used in 93 patients, including 85 patients having lumbar disk hernia and eight having cervical disk hernia, in this pain clinic. The results obtained and their significance for the pain clinic are reported here.Methods: PD was performed using the automatic nucleotome system proposed by Onik or by the Hijikata method. Various nerve blocking treatments were also performed after PD. The treatment results were assessed by attending physicians and a questionnaire sent to patients in August 1994 (response rate: 84.5%).Results: Improvement rates in activities of daily living (ADL), as assessed by the attending physicians, were 78.8% for the lumbar disk hernias and 87.5% for the cervical disk hernias. Questionnaire responses for these rates were 80.8% and 87.5%, respectively. The patients' impression was "very good" or "good" in 70.5% having lumbar disk hernias and 87.5% having cervical disk hernias (mean: 77.2%).Discussion: PD is an effective procedure for pain treatment, but the therapeutic results improve when it is used in combination with nerve blocking.
