Evaluation of internal derangement of the temporomandibular joint by using MRI and double contrast arthrographic CT

Thirty temporomandibular joints (TMJs) in 30 patients with internal derangement (anterior disc displacement without reduction) were studied with magnetic resonance imaging (MRI) and double-contrast arthrographic computed tomography (CT). It was revealed that 17 patients had osteoarthritis, and 13 patients had no bony change. MRI is beneficial not only in the depiction of the location and morphologic alterations of the disc but also in the diagnosis of avascular necrosis of the mandibular condyle. Double-contrast arthrographic CT was particularly beneficial in the detection of perforation of the disc or retrodiscal tissue, joint space changes and fibrous adhesion as well as hard-tissue changes such as cortical erosion or osteophyte. Combination of the two image modalities of MR and CT were compensation for mutual demerits, and useful to general diagnosis of temporomandibular disorders. The discs in the osteoarthritis group showed severe morphologic changes compared with another group, and perforation of soft-tissue was only found in the osteoarthritis group. The results of this study demonstrated that hard-tissue changes indicate the progression of internal derangement of the TMJ.
