Cytological diagnosis useful in definitively diagnosing mantle cell lymphoma in the lymph node-a case report-

<I><B>Background</B></I>: We report a case of mantle cell lymphoma (MCL) in the lymph node. Fine needle aspiration (FNA) cytology for suspected malignant lymphoma (ML) and fluorescence in situ hybridization using imprint smears were useful in definitively diagnosing MCL.<BR><I><B>Case: </B></I> A 65-year-old man with lymphadenopathy of the neck and inguinal lymph nodes was found in FNA cytology to have malignant lymphoma. Cytological features showed monotonous medium lymphoid cell proliferation with ovoid or cleaved nuclei. Cytoplasm varyied from scant to abundant. One to three small nucleoli were observed. Mitoses and nonphagocytic and phagocytic histiocytes were prominent. Cytological features in imprint smears were similar to those in FNA. Based on these cytological findings, a diagnosis of medium-sized malignant lymphoma was made. Fluoresence in situ hybridization (FISH) conducted using imprint smears showed chromosomal translocation, t (11; 14)(q13; p32). Combined histological, immunohistological, and flow cytometric findings led to a definitive diagnosis of MCL.<B><I>Conclusion</I>:</B> In cases where cytologic diagnosis of lymph nodes is ML, FISH analysis is crucial to definitively diagnosing ML.
