<I><B>Background: </B></I> Renal collecting duct (Bellini duct) carcinoma is a rare tumor and its cells infrequently appear in voided urine. We report a case of renal collecting duct carcinoma and discuss differential diagnosis based on cytological features.<I><B>Case: </B></I> A 56-year-old woman with macroscopic hematuria was found in voided urine cytology to have small papillary clusters and small irregular clusters composed of atypical cells with large eccentrically located nuclei and foamy cytoplasm. Large clusters with irregular nuclear overlapping and signet ring tumor cells were detected in caliceal lavage cytology. Cytological diagnosis was adenocarcinoma suggestive of renal cell carcinoma. Contrast-enhanced CT showed a hypovascular 2.5 cm mass in the upper pole of the right kidney, necessitating right nephrectomy. Histologically, tumor cells proliferated in papillary and tubular patterns and intratubular and intracytoplasmic mucins were observed. There were also carcinoma invasion with desmoplasia into the renal capsule and pelvis. Tumor cells were immunoreactive for high molecular weight cytokeratin (34β E12). The tumor was definitively diagnosed as collecting duct carcinoma.<B><I>Conclusion</I>:</B> Collecting duct carcinoma should be considered in differential diagnosis in urinary cytology when papillary clusters composed of atypical cells with intracytoplasmic mucin are observed.
