<B><I>Background</I>:</B> We report a case of proximal epithelioid sarcoma diagnosed by fine needle aspiration cytology.<BR><B><I>Case</I>:</B> A 32-year-old man referred for a tumor on the left axilla and high fever was found in fine needle aspiration to have tumor cell clusters with prominent cellular atypia and massive necrosis and frequent intraplasmic inclusion bodies. Nemerous granulocytes were also associated with tumor cells in and along cytoplasm. Granulocytes in peripheral blood increased with growth of the tumor. lmmunohistochemicaly tumor cells were positive for EMA, cytokeratin (CAM 5.2), NSE, and anti-G-CSF antigen.<BR><B><I>Conclusion</I>:</B> Cytological features of anti-G-CSF producing proximal epithelioid sarcoma do not show features typical of epithelioid sarcoma.
