<B><I>Background</I>:</B> We report invasive micropapillary carcinoma (IMP) detected in a male breast.<BR><B><I>Case</I>:</B> An 81-year-old man noticing a small firm mass just below the nipple in August 2001 was found to have a 1cm firm mobile round tumor having a smooth surface. Fine-needle aspiration cytology showed various-sized papillary clusters against a clear background. Nuclei were round and not prominent., Since no stromal cells were seen around clusters, the tumor was suspected of being malignant and resected while its histological type was unknown. Histologically, clusters composed of 10-50 cells were observed in nests and separated by stroma. Large clusters had some clear zones/vacuoles, but neither (fibro? Vacuolation/ cavitation?) nor a prominent nucelus was noted, leading to a diagnosis of IMP.<BR><B><I>Conclusions</I>:</B> Fine-needle aspiration cytology is useful in the early diagnosis of IMP.
