A study of cluster configuration in breast tumor of aspiration cytology.

<B><I>Objective</I>:</B> Some cases of breast tumor consist of small cells with weak cellular atypia. In such cases, it is difficult to diagnose whether they are benign or malignant, and so cell cluster shape is as important as individual cellular atypia. We evaluated the irregularity of cell cluster shape in both benign and malignant tumors.<BR><B><I>Study Design</I>:</B> We reviewed the shape of clusters in aspiration cytology of 16 cases (356 clusters), of papillotubular carcinoma 15 cases (251 clusters) of fibroadenomaof the breast. We examined the irregularity and shape of each cluster, length/breadth rate of each cluster, cluster size and the localization of the irregular shape in each cell cluster.<BR><B><I>Results</I>:</B> The outline of malignant cell clusters was highly irregular in most case, and slightly irregular in some. The outline of malignant clusters was rough, while that of benigh clusters was smooth. The malignant cell clusters with weak cellular atypia were arranged in a sheet-like fashion and were nearly round in shape. Cellular arrangement in each cluster was regular<BR>The length breadth rate of clusters in papillotubular carcinoma with little cellular atypia was approximately one. The configuration was multiplex branch. The irregularity of the outline was closely related to the cell overlapping and cellular density. Cell overlapping and cellular density were pronounced in the periphery of clusters in fibroadenoma and in the center in papillotubular carcinoma. Thus, the mechanism of the genesis of irregularity of the outline was different between fibroadenoma and papillotubular carcinoma.<BR><B><I>Conclusion</I>:</B> Cluster shape is of great value in the cytological differential diagnosis between papillotubular carcinoma consisting of small cells with weak cellular atypia and benign tumor.
