Alveolar soft part sarcoma is an uncommon soft tissue tumor which occurs predominantly in the extremities of young adults. We report a 12-year-old male case of alveolar soft part sarcoma arising in the left thigh.<BR>The patient visited our hospital with a chief complaint of a mass in the left thigh. Under the suspicion of malignancy, an excisional biopsy was done. The tumor was located in the vastus lateralis muscle and its size was 3 x 4 cm.<BR>Imprint cytology showed a single or loose group of large, polygonal cells with single or multiple eccentric nuclei, possessing a prominent nucleolus. The cytoplasm appeared cyanophilic and finely granulated. Some of the tumor cells had crystals and granules stained red-purple by Giemsa. These structures were diastase-resistant PAS-positive. Many naked nuclei were also present, occasionally showing a bead-like arrangement. From these cytological findings, a diagnosis of alveolar soft part sarcoma was made. Histological findings confirmed the diagnosis.<BR>Immunohistochemically, tumor cells were positive for myoglobin, supporting the myogenic nature of this tumor.<BR>In consideration of the characteristic cytoplasmic crystals and granules, Giemsa and PAS stainings are essential for the cytological diagnosis of alveolar soft part sarcoma.
