Adenomyoepithelioma (AME) of the breast is an uncommon benign tumor presenting proliferation of both glandular epithelium and myoepithelium. We report a case of AME which was difficult to differentiate from breast cancer by cytological examination of fine needle aspiration biopsy. The patient, a 58 year-old female, complained of an rregulary-shaped palpable tumor, about 1.5 cm in diameter, located in the upper medial region of her left breast. Ultrasonography of the tumor suggested a diagnosis of breast cancer. The tumor was judged to be class III based on the cytological examination due to the finding of numerous clusters of abnormal cells and because the myoepithelial cells were inconspicuous. A histological diagnosis of AME was made after excisional biopsy. Myoepithelial cells of AME are distinct from those of other benign mammary tumors; the myoepithelial cells in our case demonstrated naked and small nuclei, scattered on and at the periphery of aggregates of epithelial cells. These cells had polygonal nuclei and abundant cytoplasm faintly stained by light green stain with prominent nucleoli; furthermore, they formed clusters of various sizes. It may be possible to make a definite diagnosis of AME based on recognition of these characteristic features of myoepithelial cells.
