Morphometry of serous adenoma, borderline malignancy and adenocarcinoma of the ovary in touch smear.

Morphometry of serous neoplasms of the ovary was performed in 10 cases of adenoma, 6 cases of borderline malignancy and 24 cases of adenocarcinoma. Morphometricanalysis was performed in relation to both nuclear factors (10 factors) and structural factors of cell clustes (5 factors). Diagnostic accuracy based on the nuclear factors alone was 77%. The accuracy in adenoma, borderline malignancy and adenocarcinoma was 84, 7.8 and 92%, respectively. Seventy-one percent of borderline malignancies were diagnosed as adenoma by the judgement based on the nuclear factors alone. Diagnostic accuracy based on the structural factors of cell clusters was 73%. The accuracy in adenoma, borderline malignancy and adenocarcinoma was 55, 15 and 99%, respectively. Sixty-one percent of borderline malignancies were diagnosed as adenocarcinoma by the judgement based on the structural factors of cell clusters alone. Next, multivariate analysis was conducted by using all factors. Multivariate analysis showed that the nuclear factors were available to differentiate adenocarcinoma from the others, and that the structural factors were available to differentiate adenoma from the others. Diagnostic accuracy based on the structural factors of cell clusters was 73%. The accuracy in adenoma, borderline malignancy and adenocarcinoma was 89, 64 and 100%, respectively. Nuclear stratification, nuclei protruding outwards, cribriform pattern, marginal hyperchromasia and chromatin pattern can be especially significant factors for the differential diagnosis of the three kinds of neoplasms. In conclusion, touch smear is avialable for differential diagnosis between adenoma and adenocarcinoma in serous tumors of the ovary. However, diagnosis of borderline malignancy may be limited, especially in relation to differentiation from adenocarcinoma.
