Undifferentiated sarcoma of the liver is a rare malignant mesenchymal neoplasm that shows no specific differentiating features and has a predilection for children and young adults in the first 2 decades of life. One adult case of this tumor is reported, including imprint cytologic features.<BR>The patient was a 74-year-old man with an uncomfortable upper abdominal feeling. Serum cr-fetoprotein and carcinoembryonic antigen levels were not elevated. A computed tomographic scan and an ultrasonogram showed a multicystic mass in the right lobe of the liver. The excised tumor was soft and measured 17×15×8cm. In cross section, the tumor was well demarcated from the adjacent liver and was multicystic in appearance with mucoid material and central necrosis. Histologic sections showed chiefly sparse proliferaion of medium-sized spindle cells with myxoid and cystic degenerative stroma. In immunohistochemical examination of paraffin sections, the tumor cells were positive for vimentin alone.<BR>Imprinted cytology revealed scattered or sparsely clustered tumor cells in a myxoid background. The cells were spindle-shaped or stellate, and had comparatively abundant cytoplasm and spindel-shaped or oval nuclei with a notch and moderately increasing fine chromatin. A few multinuclear giant cells with small hyaline globules were found also.
