Correlation between vaginal cytology and serum sex hormones after administration of GnRH analog.

The relationship between cytological findings in the vagina and serum sex hormone levels after administration of GRH analog (buserelin) was investigated in 18 women with endometriosis. Buserelin was administered to the patients intranasally in a daily dose of 900μg, for a period of six months beginning on the second day of menstrual cycle. Vaginal smears and blood samples were taken simultaneously (1) in the follicular phase before treatment began, (2) 2 weeks and 4 weeks after the start of treatment, and then every four weeks, (3) in the follicular phase at three months after treatment had ended.<BR>I) The maturation index (MI) and the eosinophilic index (EI) were calculated using Papanicolaou stained vaginal smears.<BR>II) Simultaneous serum FSH, LH and estradiol (E<SUB>2</SUB>) levels were determined by RIA.<BR>Indexes and the levels during and after treatment were compared with those before treatment.<BR>I) Superficial cells (SFs) and/or eosinophilic cells in smears obtained during treatment were prominent at two weeks. From the fourth week onward, intermediate cells (IMs) and/or basophilic cells predominated, and few parabasal cells were seen.<BR>SFs decreased siginificanity from the 16 th week (p<0.05). IMs were significantly higher at 16 and 24 weeks. EI decreased significantly from the 12th week onward.<BR>II) The changes in hormone levels were as follows: FSH persisted at the follicular phase level. LH was maintained at the basal level of the follicular phase. E<SUB>2</SUB> was temporarily higher at the second week, and decreased from the fourth week onward below the basal level of the follicular phase, with significantly lower values at 8, 12 and 24 weeks.<BR>These changes returned to their pretreatment state by three months after the discontinuation of administration.<BR>III) Relation between MI, EI and hormone levels: There were no correlations between the indexes and the levels. However, it was fairly evident that the changes in SFs and EI were roughly correlated with those of the E<SUB>2</SUB> levels.<BR>These findings suggest that MI and EI might serve as useful parameters for evaluating the effects of GnRH analog.
