Report of a case of pleomorphic lobular carcinoma of the breast.

In 1982 Dixon et al. classified invasive lobular carcinoma (ILC) into four categories, one of which was pleomorphic lobular carcinoma (PLC) and characterized, as the name implies by its bizarre nuclei. The few reports that have appeared in the literature to date have mainly been based on histological findings. We investigated the cytologic criteria for this entity based on aspiration biopsy cytology (ABC) in a 42-year-old woman with a 4.0×4.4 cm mass in the left breast suspected of being cancer. The aspirate revealed scattered and clustered cells with a low N/C ratio. The tumor cells were polyhedral-shaped and possessed granularrich cytoplasm which stained with light green. The hyperchromatic nuclei were medium to large in size and oval or irregular in shape, and a few multinucleated tumor cells were noted. Differential diagnosis was possible based on additional findings of Indian-file arrangement and granular cytoplasm. PLC, with its worse prognosis, warrants careful study and detection by cytological means prior to surgical intervention.
