A case of ovarian dysgerminoma-Its ascitic cytological findings.

We treated a patient with ovarian dysgerminoma in whom tumor cells appeared in ascites. The patient was a 45-year-old woman who visited Takasago Municipal Hospital because of pain and a sensation of fullness in the lower abdomen. Ovarian tumor was suspected, and bilateral oophorectomy and simple total hysterectomy were performed. The tumor was the size of a new-born's head and was solid. Furthermore, 1100 m<I>l</I> of ascites was observed. Tumor cells with a large nucleus including an enlarged nucleolus appeared as sheets in ascitic cytology. The cytoplasm of tumor cells was pale and clear; the tumor cells were well defined and intussusception was not found. Histologicallly, a typical picture of dysgerminoma was observed. Dysgerminoma is characterized by the so-called 2-cell pattern in which tumor cells and small lymphocytes appear in imprint cytology and aspiration cytology; however, the 2-cell pattern was not clear in this patient in ascitic cytology. Although differentiation from adenocarcinoma and malignant lymphoma is required cytologically, we consider the diagnosis might be relatively easy even when the 2-cell pattern is not clear but the appearance of typical dysgerminoma is observed in ascites, as in the present case.
