A case of localized mesothelioma that followed a 26-year course ending in death is reported. A 38-year-old insurance saleswoman underwent right middle lobectomy for a pleural giant tumor, localized mesothelioma, in 1963. Local recurrence was found in her right pleura with severe hypoglycemia in 1975, followed by 5 recurrences with hypoglycemia and 5 resections of the tumor in total. She died in 1989, due to recurrence of the tumor, which showed bilateral lung metastases and retroperitoneal invasion. Cytopathologic study of the tumor revealed certain characteristics of malignant transformation: increased cellurarity and atypism of sarcomatous spindle cells, and the increasing appearance of epithelial cells with advance of the course. On nuclear measurement study of the tumor cells, the cells obtained in 1988 showed significant amplification compared with those obtained in 1978 and in 1984 (p<0.01)
