Cellular atypia in transitional cells associated with urolithiasis.

Spontaneously voided urine was cytologically reviewed for atypical transitional cells in 27 samples of 22 patients with urolithiasis who consulted our urology clinic from September 1989 to August 1990, and compared with cases of low and high grade transitional cell carcinoma. The purpose of the pres-ent study was to elucidate the problems of differential diagno-sis between reactive atypia of the transitional cells seen in urolithiasis and transitional cell carcinoma of the urinary tract.<BR>The results were as follows. There was no significant relation between cellular atypia and the location or size of stones, but calcium phosphate could be found in 53.3% of the cases of urolithiasis.<BR>Cytologically, the size of nuclei was somewhat smaller in reactive transitional cells in urolithiasis. The dispersed cells in urolithiasis showed no hyperchromatic nuclei. These findings were considered to make urine cytology screening more effective in the differential diagnosis between urolith-iasis and transitional cell carcinoma.
