A study of endometrial cytology with endocyte.

The reasons for the discrepancy between the cytological diagnosis (ENDOCYTE was used as equipment) and the pathological diagnosis were studied for 54 cases of proliferative phase of endometrium (EM), 41 cases of secretory phase of EM, 33 cases of endometrial hyperplasia and 49 cases of endometrial cancer for these four and half years. And also the difference between endometrial hyperplasia and well differentiated endometrial cancer (G<SUB>1</SUB>) was studied at the view point of cytology. The results obtained were as follows;<BR>1. Main reason (75% of cases) of overdiagnosis for proliferative phase of EM was due to the overlapping of the stromal cells. The irregularity of the nuclear size or the dysproportion of the nucleus were recognized, which were caused by the overlapping stromal cells.<BR>2. At the secretory phase, not only slightly lager nucleus and the clear nucleoli, but also unevenness of the distance of each nucleus were sometimes shown. And in 47% of cases, there were overlapping of the stromal cells as same as normal EM. Sometimes the stromal cells became larger.<BR>3. At the cases of endometrial hyperplasia, following results were pointed out as the cytological findings.(1) Round or oval shaped nucleus.(2) Homogenous chromatin pattern.(3) No shortening of the distance of each nucleus and no change of polarity, even if there were overlapping.(4) Normal EM cells were seen in almost all cases, but sometimes atypical cells appeared.<BR>4. At the cases of G<SUB>1</SUB>, the difference compared with endometrial hyperplasia were as follows;(1) There were irregular shaped nucleus except for round or oval shaped one.(2) Small sized nucleoli were sometimes observed.(3) Unevenness of the distances of each nucleus.(4) Palisade like arrangment.(5) There were small amount of normal EM cells. These five findings were thought to be important as the differential diagnosis between endometrial hyperplasia and G<SUB>1</SUB>.
