Cytologic findings of malignant cells obtained by fine needle aspiration of primary lung cancer producing alpha-fetoprotein. A case report.

A case of primary lung cancer producing alphafetoprotein (AFP) diagnosed by fine needle aspiration cytology was reported.<BR>A 70-year-old woman consulted her physician because of a persistent cough and right chest pain. Chest X-ray revealed a large mass shadow in the right upper lung closed to the mediastinum. Serum level of AFP increased to 11, 893 ng/m<I>l</I>.<BR>Tumor cells obtained by fine needle aspiration had an overlapping or flat arrangement. These cells had abundant granular cytoplasm and irregularly shaped nuclei of various size. Hyperchromasia was remarkable and chromatin granules were coarsely distributed. Single prominent nucleolus was round in shape. By PAS stain, some tumor cells showed positive reaction. The immunoenzyme labelling technique with PAP method showed AFP in some tumor cells.<BR>Clinical examination of liver and the other abdominal organs, digestive organ and ovaries showed no abnomalites, so we suggested this lung tumor as a primary adenocarcinoma producing AFP. And, by needle biopsy of this tumor and autopsy, this tumor was confirmed as primary lung cancer of poorly differentiated adenocarcinoma.
