<B><I>Objective</I>:</B> Pleomorphic adenoma is known to have diverse histological and cytological features consisting of both epithelial and mesenchymal components. The histological features of this tumor result from a variety of morphological appearances of neoplastic myoepithelial cells. To evaluate cytologic characteristics of neoplastic myoepithelial cells in pleomorphic adenomas, we compared cytological features of these cells to histological findings.<BR><I><B>Study Design:</B></I> We reviewed cytology smears of 30 cases histologically diagnosed as pleomorphic adenoma of the salivary glands (parotid gland, 22 cases; submandibular gland, 8 cases) from the files of Tokyo Medical University Hospital. Histological specimens were studied immunohistochemically to document their myoepithelial nature.<BR><B><I>Results</I>:</B> Neoplastic myoepithelial cells are classified into epithelioid, clear, plasmacytoid, and spindle/stellate. Special types include chondroid, metaplastic squamous, large bizarre, basaloid, and adipocyte-like. By comparing histological features, we identified neoplastic myoepithelial cells in cytological smears. Cytologically, plasmacytoid and spindle/stellate cells are easily recognized, whereas clear and adipocyte-like cells are difficult to identify.<BR><B><I>Conclusion</I>:</B> A comparison of cytological features with histological findings was helpful in understanding the cytological diversity of neoplastic myoepithelial cells in pleomorphic adenoma. For cytological diagnosis of pleomorphic adenoma, it is important to understand the morphological diversity of neoplastic myoepithelial cells.
