<B><I>Background</I>:</B> Conization is generally agreed to be effective in treating cervical intraepithelial neoplasia grade III (CIN III), especially in patients wishing to preserve fertility.<BR><B><I>Case</I>:</B> A 36-year-old nulligravida was treated with conization for CIN III at the age of 31. Nineteen months later, CIN III had recurred, so the conization was redone. New vaginal intraepithelial neoplasia grade III (VAIN III) developed 3 years after the second conization, although the patient had clear margins in both operation specimens. High-risk HPV type 16 was identified in this case.<BR><B><I>Conclusion</I>:</B> Conization with clear margins is generally adequate in treating CIN III, but patients at highrisk for HPV infection require especially careful follow-up.
