Comparison of telecytological diagnostic accuracy between still images and glass slides and difference of still images transmitted among hospitals

<B><I>Objective</I>:</B> We evaluated the diagnostic concordance of individual observers between video monitoring and glass slide diag nosis of cytological specimens, and compared cytological images transmitted among hospitals.<BR><B><I>Study Design</I>:</B> (1) After monitor diagnosis of cytology, direct diagnosis was made on the same cells diagnosed on monitors twice after 1 month.(2) The cyto-and pathotechnologists transmitted still images of sputum, pleural effusion, and fine-needle breast aspiration cytology used both distributed and diagnosed specimens at each hospital. Still images were analyzed for the number of still images, magnification, and staining quality.<BR><B><I>Results</I>:</B> (1) Crude agreement of classification and pathological diagnosis with still images was 67.3 to 83.7% and 40.8 to 67.3% the first time and 81.4 to 95.3% and 512 to 72.1% the second time. Crude agreement of classification and pathological diagnosis in direct diagnosis was 73.5 to 91.8% and 42.9 to 75.5% the first time and 74.4 to 97.7% and 58.1 to 81.4% the second time.(2) Still images were transmitted at 2 to 5, 10×to 60×magnifications of the distributed specimens, and at 2 to 6, 10×to×60 used specimens diagnosed by cyto-and pathothechnologists at each hospital.<BR><B><I>Conclusions</I>:</B> Telecytology is efficient when cyto-and pathotechnologists cannot discuss cytological diagnosis with cytopathologists. Correct diagnosis, howerver, presupposes previous agreement between cytotechnologists and cytopathologists on appropriate staining, the number of transmitted images, and magnification for diagnosis.
