A case of primary malignant melanoma of the lung associated with von Recklinghausen's disease

<B><I>Background</I>:</B> We report a case of primary malignant melanoma of the lung associated with von Recklinghausen's disease (neurofibromatosis type 1: NF1).<BR><B><I>Case</I>:</B> A 45-year-old woman diagnosed as NF1 in childhood, was found in chest radiography and computed tomography (CT) to have a coin lesion in the right lower lobe of the lung. CT-guided fine-needle aspiration cytology showed atypical malignant melanoma cells with dark brown granules. Multinucleated giant cells, fusiform or heteromorphic cells, and intranuclear inclusion bodies were also seen. We suspected malignant melanoma from these cytologic findings, conducting immediate right lower lobectomy of the lung and mediastinal dissection. Postoperative histological findings showed junctional change at the bronchial epithelium. Preoperative clinical findings and intraoperative findings showed no melanoma lesion except in the lung. Based on these findings, we diagnosed primary malignant melanoma of the lung. Nine months postoperatively, the patient died of multiple brain metastases.<BR><B><I>Conclusion</I>:</B> Although cancers including malignant melanoma are well known to be associated with NF1, primary malignant melanoma of the lung is rare. Association with NF1 is even rarer. We diagnosed primary malignant melanoma of the lung from histological findings. We wish to emphasize that characteristic cytologic findings were useful in preoperative diagnosis of malignant melanoma.
