<B><I>Objective</I>:</B> We evaluated cytological findings in benign cases to increase the accuracy of endometrial cytology for hyperplasia.<BR><B><I>Study Design</I>:</B> We selected 87 biopsied cases diagnosed as suspicious (possible hyperplasia).<BR>Histological findings were classified as benign in 31 (14 atrophic endometrium), 3 proliferative phase, 6 secretory phase, 5 irregular proliferation, polyp, and 56 hyperplasia (14 simple and 42 complex). Each case was compared for the appearance and frequency of abnormal clusters and of metaplastic epithelium.<BR><B><I>Results</I>:</B> 1) Appearance and frequency of dilated glands were high in hyperplasia, secretory phase, irregular proliferation, and polyp.<BR>2) The appearance of clumps of irregularly ragged glands in atrophic endometrium and the proliferative phase was comparatively high.<BR>3) The appearance and frequency of clumps with high densities of glands in benign were low.<BR>4) In the appearance of metaplasia, no significance was between benign and hyperplasia<BR><B><I>Cases</I>:</B> Appearance of metaplasia were high in atrophic endometrium and the proliferative phase.<BR><B><I>Conclusion</I>:</B> The appearance and frequency of abnormal clusters and of metaplastic epithelium increased the diagnostic accuracy of hyperplasia in endometrial cytology.
