<B><I>Background</I></B>: A case of pleuro-pulmonary cryptococcosis with massive pleural effusion is reported.<BR><B><I>Case</I></B>: A 67-year-old man was admitted to our hospital because of low grade fever and systemic edema. He had a past medical history of diabetic chronic renal failure and had undergone hemodialysis for 6 months. Chest-X-ray film showed abnormal shadows in the left lower lobe and massive pleural effusion. Cytology of pleural effusion was positive for cryptococcus neoformans, whereas it was not detected by culture examination.<BR><B><I>Conclusion</I>:</B> Recently the frequencies of AIDS and medical graft have increased in Japan. This might have led to an increase in the frequency of opportunistic infections including cryptococcosis in such patients. Cytology of pleural effusion is very useful for detection of pleuro-pulmonary cryptococcosis.
