The present study was designed to elucidate cytologic features useful for detecting carcinoma cells with small nuclei, and to differentiate them from benign lesions.<BR>The following cytologic findings were identified at the indicated frequency, nuclear pleomorphism (39%), intracytoplasmic lumen (ICL)(41%), individual solitary epithlial cells (78%), structural atypia (cribriform, trabecular pattern)(31%), absence of the two cell pattern (90%), and absence of the immunocytochemical two cell pattern (96%), These findings were variable among histologic types. Cellular atypia, such as nuclear pleomorphism and ICL, was relatively less frequent in papillo-tubular carcinoma (13%, 13%), and was frequently identified in scirrhous carcinoma (50%, 50%) and solid-tubular carcinima (57%, 57%). The trabecular pattern was often identified in scirrhous carcinoma (42%), and solitary epithelial cells were frequently identified in solid-tubular<BR>carcinoma (57%), Loss of the two cell pattern was observed in all histologic types including papillo-tubular carcinoma, and constituted an important finding. Immunocytohemical detection of myoepithelial cells is a useful method, though the presence of myofibroblasts or residual myoepithelial cells in non-invasive ductal carcinoma should be interpreted wiht caution.
