A postmenopausal tuberculous pyometra case whose endometrial smear was useful for diagnosis.

We report the rare case of 69 year old postmenopausal woman with tuberculous pyometra whose endometrial smear was useful for diagnosis. Cervical smear showed no abnormal findings. Before washing of the uterine cavity, an endometrial smear exhibited a number of lympyhocytes, epithelioid cell clusters, Langhans giant cells and the background was watery with protein droplets. After washing the uterine cavity, an endometrial smear showed essentially the same findings, except for a relatively clean background. Histology indicated a chronic granulomatous inflammatory disease such as tuberculosis. Acid fast bacterial stainings were positive, detection of mycobacterium DNA was positive and the tuberculin reaction was positive. We therefore diagnosed postmenopausal tuberculous pyometra. Pyometra is a relatively rare condition. It is this important in making the diagnosis, that tuberculous pyometra be recognised and an endometrial smear is occasionally a useful diagnostic procedure.
