Comparative analysis of endometrial cytology in endometrial and ovarian clear cell adenocarcinomas.

Endometrial cytologies of 7 cases with clear cell adenocarcinoma (CCC), of endometrial or ovarian origin, were comparatively analyzed in addition to clinical and histopathological features.<BR>1) Frequencies of CCC among whole endometrial and ovarian carcinomas were 1.8% and 14.4%, respectively.<BR>2) The lesion was localized coexisting with normal endometrium in 3 out of 4 cases of endometrial CCC.Solid, solid with glandular and tubulocystic patterns were found in 1, 2 and 1 case, respectively, whereas in ovarian CCC the histopathological appearance was rich in variety and had a papillary pattern.<BR>3) The common cytological features of CCC were large cells and nuclei with anisonucleosis, prominent nucleoli and abundant clear cytoplasma.<BR>4) In endometrial CCC, there were many clusters which exhibited large aggregated and large cellular sheets with irregular margins. Tumor diathesis was also a prominent feature.<BR>5) In ovarian CCC, there were clusters which exhibited small mirror ball patterns with smooth margins, and small cellular sheets on a clear background.<BR>6) The essential points for differential diagnosis between endometrial and ovarian CCCs are the background features, coexistence of normal endometrial cells, the maximal diameters of cellular clusters and sheets (borderline: 0.5mm), cluster margins and the mirror ball pattern.
