A case of severe cryptosporidiosis, which developed in a 24-year-old type A hemophiliac with AIDS, is reported.<BR>At the age of 21 years, <I>Pneumocystis carnii</I> pneumonia occurred, and the diagnosis of AIDS was made.At 24, severe diarrhea appeared, which was resistant to routine antibiotics. Stool examination revealed tiny <I>Rhizopoda</I>-like protozoans of unknown origin.Biopsy specimens of the rectal mucosa disclosed numerous cryptosporidian organisms attached to the mucosal epithelial cell surface.Cytologic and electron microscopic examinations of duodenal aspirates revealed numerous merozoites or sporozoites as well as trophozoites of <I>Cryptosporidium</I>. Cryptosporidian oocysts were also detected in the sputum smear.<BR>Cytologically, Giemsa stain and Kinyoun's acid-fast stain were useful for making the diagnosis in our case.Since <I>Cryptosporidium</I> is a tiny protozoan, careful attention is required in screening stool samples from patients with severe diarrhea, especially in those from immunocompromised hosts such as AIDS patients.
