A case of mesodermal mixed tumor(MMT) of the ovary. Cytological and immunohistochemical study.

A primary ovarian tumor with carcinomatous and sarcomatous elements intimately mixed together is called mesodermal mixed tumor (MMT).Only 23 cases have been reported to date in Japan. We had an opportunity to treat an MMT originating from the ovary and studied it cytologically, pathologically and immunohistopathologically.This case was a 54-year-old gravida 5 para 4 performance status grade 4 woman with a huge mixed density tumor in the abdomen and right pleural effusion.Cytology of the effusion revealed adenocarcinomatous cells.The patient underwent exploratory laparotomy and chemotherapy consisting of etoposide and platinum preparations (carboplatin).Spindle-round sarcoma cells were detected by inprint cytology of the tumor.<BR>Pathological examinations showed coexistence of epithelial and mese nchymal components.The former showed swollen, nuclei varying in size and having prominent nucleoli. The cytoplasm of the latter was basophilic, the margin being obscure.The cells showed positive immunostaining with CA 125, EMA in the epithelial component, and with actin, vimentin and myoglobin in the mesenchymal component. This case was diagnosed as having a homologous type ovarian tumor, MMT, on the basis of these histological examinations.
