To determine the specific features of prostatic cancer cells in urine, we compared their morphological characteristics with those of other metastatic adenocarcinoma cells. We examined tumor cells in urinary cytology specimens from 39 patients with adenocarcinoma.
Twenty patients had prostatic cancer (group A).The clinical stage was stage C or higher, in 18 cases, stage B 2 in one case and unclear in the other.Clinical staging was in accordance with the general rules for clinical and pathological studies on prostatic cancer in Japan. The grade of differentiation of the prostatic adenocarcinomas was moderate or poor in all but the unclear case.
The other adenocarcinomas (Group B) included 6 colon cancers, 3 renal cell carcinomas, 4 urinary bladder adnocarcinomas, 2 uterine coups cancers, 2 stomach cancers, one tubal cancer and one ovarian cancer.
Adenocarcinoma cells in the urine were classified into 4 types, i.e., small, round cells having a high N/C ratio and deviated nulei measuring under 9μ in the longest diameter, referred to as type I a, round cells larger than type I a classitiedds type I b, the columnar tumor cells designated type II, and adenocarcinoma cells of signet ring cell type referred to as type III.Type I a carcinoma cells were found in 10 of the 20 prostatic cancers, but only one was group B.
Adenocarcinoma cells having nuclei measuring under 7μ in the longest diameter, were found in 5 cases in group A but in none in group B.
Accordingly, it appeared that adenocarcinoma cells of type I a or having the very small nuclei mentioned above may represent prostatic cancer cells in the urine.
