A case of pineocytoma is presented with its cytology, histopathology, immunohistochemistry, electron microscopy and proliferative activity, which were studied using bromodeoxyuridine (BrdU) and proliferating cell nuclear antigen (PCNA).<BR>A 40-year-old mancomplaining of headache and hydrocephalus was referred to Kochi Medical School Hospital. A CT scan and MRI demonstrated a tumor measuring about 2.5cm in size in the pineal region.After placement of a verltriculoperitoneal shunt, tumor resection was performed. Imprint cytology showed small cells with relatively high N/C ratios arranged in small clusters. A sheet of tumor cells showed relatively scant cytoplasm with clear borders, round to oval nuclei of uniform size, containing fine chromatin and small nucleoli.Rosette-like structures were occasionally found.Histologically, the tumor was composed of abundant small cells with scant eosinophilic cytoplasm, separated by a thin connective tissue stroma.Pineocytomatous rosettes and occasional Homer Wright rosettes were identified.Immunohistochemical staining with the ABC method showed that the tumor cells were positive for synaptophysin, less frequently positive for neurofilament, GFAP and S-100 protein. Ultrastructural studies demonstrated that the tumor was composed of undifferentiated primitive cells with scant cytoplasms and differentiated cells with various organeUes including annulate lamellae, microtubules and elongated cell processes occasionally forming synaptic structures which indicated neuronal differentiation.<BR>The BrdU positive tumor cells comprised about 3%, the PCNA positive cells about 5%.The proliferative activity of a pineocytoma is not significanty lower than those of other benign brain tumors.
