A case of malignant lymphoma of the breast, characterized cytologically by variegation with an admixture of mature lymphocytes, is reported. The patient was a 31-year-old married woman with a right breast lump. A 3×3cm tumor was palpated in the A area of right breast, but no distinct tumor shadow was demonstrated on mammography. An echogram showed a tumorous lesion with an irregular border and nonhomogenous internal echo.
Aspiration cytology disclosed atypical lymphoid cells of variable size with round to oval and frequently cleaved nuclei and prominent but dispersely scattered nucleoli intermingled with mature small lymphocytes.Histological diagnosis of the resected tumor was diffuse lymphoma, mixed cell type, and B-cell markers (L 26 and MB 1) were positive in the tumor cell cytoplasm.Four ipsilateral axillary lymph nodes were enlarged with similar lymphomatous growth.
Cytological diagnosis of malignant lymphoma of the breast is not always easy because of the frequent coexistence of mature small lymphocytes in the aspiration smear specimens. However, the atypia of each tumor cell facilitates the correct diagnosis.Differential diagnoses are also discussed.
