A rare case of so called cancer metastasis in remote adenoma showing metastasis of breast scirrhous carcinoma in bilateral thyroid adenoma is reported with a review of the literature. Imprint cytology as well as histological features of the metastasized cancer cells are particularly mentioned.<BR>A 53-year-old female underwent a radical operation for breast scirrhous carcinoma 4 years ago. During the postoperative observation period, the patient developed nodular swellings in the anterior neck, and enucleation of two nodules in bilateral lobes of thyroid was performed under the diagnosis thyroid adenoma.<BR>Imprint cytology made during the operation revealed bilaterally that the specimen mostly consisted of microfollicular cell clusters of follicular adenoma type which were slightly larger in size than normal follicular cells, and that large atypical cells were present among these adenoma cells. The atypical cells, which were of adenocarcinoma type forming closely packed clump of cells, showed intracytoplasmic lumina (ICL) demonstrated with alcian blue-PAS staining after destaining the Papanicolaou preparations.<BR>Histologically, both adenomas were of follicular type and cancer cells were scattered among the adenoma follicles in the center and under the capsule of the adenoma forming nests of tubular structures with capsular as well as vascular invasion. The presence of ICL, demonstrated with alcian blue-PAS staining, further confirmed by electron microscopy, suggested metastasis of breast cancer of scirrhous type in particular in the thyroid follicular adenomas.
