Placental-site trophoblastic tumor - A case report.

A case of placental-site trophoblastic tumor arising in the uterine cervix is reported.<BR>The patient, a 27-year-old female, experienced irregular genital bleeding 9 months after a spontaneous abortion. A cervical mass was noted and the biopsy specimen was suggestive choriocarcinoma. The urinary hCG was 2, 000 mIU/m<I>l</I>. Total transabdominal hysterectomy was performed.<BR>Cervical smears showed single or small-clustered atypical cells having abundant cytoplasm and one to two nuclei. Nucleoli were not prominent. No syncytial cell was noted in the specimen.<BR>Macroscopically, the tumor, measuring 5×5×3 cm, was located in the posterior part of the cervix and parts of the surfase were necrotic and hemorrhagic.<BR>Histopathologically, tumor cells were large, polygonal or rounded in shape, rather well demarcated and infiltrated into the myometrium in cords, clusters or singly. They were not arranged in biphasic pattern as in choriocarcinoma.<BR>Immunohistochemically, about half of the tumor cells were positive to hCG and hPL.<BR>The cytological characteristic of the PSTT is the presence of intermediate trophoblasts and the lack of cytotrophoblasts and syncytiotrophoblasts.<BR>The most important criterion for the cytological diagnosis of the PSTT appears to be the lack of the syncytial cells.
