A case with ovarian tumor which showed malignant transformation during 8 years was presented. A 60-yearold housewife received total abdominal hysterectomy and bilateral adenexectomy because of uterine myoma and bilateral ovarian cyst. The histopathology of both ovaries was mucinous cystadenoma. Theirafter abdominal tumor recurred and was resected in each of 4 years and 7 years later. Either tumor was considered to be mucinous cystadenocarcinoma (low grade of malignancy). Cytology of the tumor revealed round or cylindrical atypical cells with dirty background. These atypical cells showed slightly piled up cluster, anisokaryosis, slight hyperchromasia with distinct nucleolei. Eight years later the tumor recurred. Exstirpation of the tumor was imcomplete due to invasive growth to the pervic wall. Histopathology of the tumor was adenocarcinoma (probably mucinous type) and in cytological specimen there were many atypical grandular cells with apparent hyperchromasia and prominent nucleolei. The patient died of the disease. Autopsy revealed no other primary site of the adenocarcinoma, and it was concluded that the fatal adenocarcinoma developed from the ovarian cystadenoma of 8 years ago.
