Abnormal cervical cytology in patients with submucous myoma.

Submucous myoma is said to comprise 2.4% of all uterine myomas. One of our patients with abnormal cervical cytology had portioerosion and submucous myoma simultaneously. This prompted us to study which of these two lesions was the predecessor of the abnormal cells seen on cytology. During the past two years, we analyzed five cases of submucous myoma. Abnormal cells with hyperchromatic nuclei were identified as repair cells. By examining cervical smears and microscopic sections of resected uteri, it was found that these abnormal cells originated from the outer layer or the submucous myoma. This outer layer consisted of inflammed endometrium and granulation tissue. Disturbed circulation and inflammatory stimuli induced both degenerative and regenerative changes on the covering endometrium which resulted in the shedding of damaged endometrial cells and repair cells.<BR>In the future, endometrial smears will become more popular in screening for endometrial cancer. As repair cells resemble endometrial cancer cells, we must take caution to distinguish between these two cell types. If repair cells are found in cervical or endometrial smears, one must be sure to include submucous myoma, which grows silently in the uterine cavity, in the differential diagnosis.
