A procedure was developed for obtaining both cytologic and histologic specimens at the time of ultrasonically guided liver aspiration biopsy using a 22-gauge needle, 0.4 mm in inner diameter, from patients suspected of hepatic malignancy. The fine needles and syringes used were heparinized just before use for avoiding blood coagulation of samples obtained. This heparinization enabled to centrifuge these specimens so as to make good smears containing much abundunt tumor cells, and to obtain small tissue specimens. Histological specimens were obtained using thus heparinized needle and fixed in 2.5% glutaraldehyde, followed by 1.0% osmium tetroxide fixation, and embedded in epoxy Epon resin. Ultrathin sections were made and usually stained according to an Azan-Mallory method. In 21 patients suspected of hepatic malignancy, the combination of cytological and histological examination provided its correct diagnosis in 9 cases without false positive cases. Therefore, this procedure developed in our clinic can be considered to be of most reliable diagnostic method for hepatic malignancy.
