Keratin proteins in the cancerous and non-cancerous mucosa of the gallbladder were examined using of an immuno-peroxidase technique in connection with the relative high incidence of squamous metaplasia in carcinoma of this organ.<BR>Keratin proteins were found in 75%(15/20) of the cancerous mucosa of the gallbladder, but not detected in the non-cancerous mucosa with chronic cholecystitis.<BR>Keratin proteins were also found in 6%(2/30) of gastric cancer, in 12%(3/24) of colon cancer, in 75%(6/8) of pancreatic cancer and in 100%(3/3) of malignant pleural mesothelioma, but not found in the mesothelial cells in ascites.<BR>In addition, keratin proteins were weakly positive in some cells of intestinal metaplasia around the gastric carcinomas and surrounding mucosa of the colonic carcinomas.<BR>The stromal cells derived from mesoderm were negative for this proteins.<BR>In the same materials, carcinoembryonic antigen (CEA) was present in all carcinoma of the gallbladder, colon and pancreas and in 90%(27/30) of gastric carcinomas but not in 3 cases of malignant pleural mesotheliomas. The mesothelial cells in ascites from 6 patients with liver cirrhosis showed negative staining for CEA.<BR>This study indicates that the presens of keratin proteins and CEA in the cells of bile and ascites is highly indicative of cancer of the gallbladder, biliary tract and pancreas.
