Significance of Bile juice Cytology in Obstructive Jaundice

Exfoliative diagnostic cytology of the biliary tract in 25 patients with obstructive jaundice was examined by means of bile juice cytology and bile duct washing cytology.<BR>Relationship among the primary lesion, cell collecting method and cytological finding was studied.<BR>The results were summarized as follows.<BR>1. Cancer cells were observed in 9 of 12 cases (75%) with choledochal cancer, one of 5 cases (20%) with pancreas cancer and 2 of 4 cases (50%) with gallbladder cancer. No malignant cells were observed in the cases with recurrence of gastric cancer and retroperitoneal tumor.<BR>2. Cytology of bile juice from PTC drainage was performed in 14 cases and cancer cells were observed in 8 cases of them (57%). Cytology of PTCD washing fluid was performed in 9 cases and 4 cases of them were positive in cytology (44%).<BR>3. In 12 cytological positive cases, 7 cases were diagnosed as postive in the first cytology, and 5 cases were diagnosed as positive in the 2nd or more later cytology.<BR>4. PTCD catheter tip were placed near the lesion in 17 cases, and 9 of them were positive in cytology. In 6 cases the PTCD catheter tip were far from the lesion, and 3 of them were positive in cytology.
