Thirty-nine children (28 with spastic diplegia and 11 without motor deficits) who had been born preterm underwent evaluation using Frostig developmental test of visual perception and magnetic resonance imaging. We obtained T2-weighted and T1-weighted images in the coronal and transaxial planes. In T2-weighted images, the topographic distribution of periventricular hyperintensity (PVH) were examined. T1-weighted images were used for estimating the degree of parieto-occipital white matter thinning. We classified it into three groups and examined the correlation between them and the results of Frostig test (PQ). PVH was seen in 28 diplegic and 9 children without motor deficits. PVH extended to the corticospinal tracts within the frontal corona radiata in all diplegic children, whereas it was localized to the parieto-occipital region in 9 children without motor deficits. In addition, the degree of white matter thinning correlated with PQ. Parieto-occipital white matter damage in preterm children may result in developmental deficits of visual perception. MRI may be used to predict developmental deficits of visual perception as well as motor deficits and cerebral visual disturbance in preterm children. Early recognition of developmental deficits of visual perception in preterm children, especially those with spastic diplegia, may be of great importance, since it would allow the initiation of occupational therapy to correct them.
